questions and rank how your organization is doing with regard to each question.
Rank your answer on a scale of one / five where one indicates
not very well and a five indicates that your company is doing very well in
that area.
At the end of the fifteen questions you
will be presented with a score and some instructions on what your score
indicates and you will have an opportunity to provide us with your contact
